“Notice of Non-Profit Service Scope and Limitations” Agreement
Thank you for considering our community nonprofit for emotional support and spiritual care. As a nonprofit organization, we are dedicated to offering compassionate support to underserved, minority, and vulnerable populations. Please review our service details and limitations to understand the scope of support we provide:
1. Community-Driven Nonprofit: We are a community-based nonprofit organization committed to fostering emotional support and spiritual care in a safe, inclusive environment. Our primary goal is to enhance well-being, connection, and resilience within our community.
2. Service Providers: Our support team comprises trained community peers, chaplains, and professional students developing their skills in emotional support and spiritual care. These individuals are dedicated to providing empathetic, nonjudgmental support but may not be licensed mental health professionals. All providers are trained to offer guidance, active listening, and compassionate presence.
3. Service Limitations:
No Diagnosis or Treatment: We do not diagnose, treat, or prescribe medication for any mental or physical health disorders or needs.
No Mental Health Therapy: Our services are not a substitute for psychiatric, psychological, or psychotherapy treatment. If you require these services please refer to our resource page to find professionals in your local area who can provide licensed care.
Emergency Resources: If you are experiencing suicidal thoughts or are in need of emergency mental health treatment, we encourage you to contact the National Suicide Prevention Lifeline or other mental health hotlines listed on our resource page. For any urgent medical needs, please contact your local emergency number (such as 911) or go to the nearest emergency facility.
4. Legal Disclaimer: By choosing our services, you voluntarily agree to the terms outlined in this Notice of Service Scope & Limitations Agreement.
You acknowledge and accept that our nonprofit’s support and services is provided “as-is,” without guarantees, and is limited to emotional support and spiritual care from trained peers, chaplains, and professional student providers.
By choosing our services, you acknowledge that they are not a substitute for licensed mental health or medical care. You voluntarily agree to receive support during your time of need and release our organization from any liability for perceived or actual omissions or errors related to serious medical conditions, endangerment claims, or professional liability.
Furthermore, you agree to indemnify and hold harmless our organization, staff, and volunteers from any claims, damages, or legal expenses resulting from your use of our services. For emergency needs, please contact your local emergency services.
